P0 Leeds Beckett University (UK) LBU

Leeds Beckett University is a higher education corporation that provides access to education for a wide range of learners, promoting cultural awareness and diversity through its curriculum. Through the partnerships with schools, colleges, further education colleges and employers, Leeds Beckett enables those with a willingness to learn to progress to higher education and a lifetime of learning.

Getting Sorted has a strong reputation for its work with children and young people (CYP) with Type 1 diabetes (T1DM). Getting Sorted specialises in participatory research with CYP with T1DM, their families and healthcare professionals. The aim of their work is to enable CYP with T1DM achieve their optimal health and to develop, influence and enhance service re-design and service improvement in the UK and beyond.

 
PROJECT PARTNERS
P1 Foundation Hannoversche Kinderheinlanstalt (DE) HKA

The HKA has a prominent position amongst the non-governmental institutions for the support and care of sick, underprivileged and handicapped children in Germany. The associated Diabetes Centre for Children and Adolescents with ca. 700 children is the largest paediatric diabetes centre in Germany. The multidisciplinary team has a high level of diabetes specific expertise based on long experiences in in- and outpatient care, teaching and training activities as well as numerous national and international clinical projects to improve diabetes care in children.

 

P2 Hannover Medical School (DE) MHH

The Department of Medical Psychology at Hannover Medical School comprises six psychologists, two of whom have been specialists in paediatric diabetology for more than three decades. They have a wealth of experience in the development of teaching and training activities for diabetes multi-disciplinary team (MDT) members and the evaluation of diabetes education programmes. They work closely with P1, the Diabetes Centre for Children and Adolescents (HKA).

 

P4 APDP Diabetes Portugal (PT) APDP

APDP is the largest outpatient diabetes clinic in Portugal (for type 1 and 2). It has clinical staff that provides separate and dedicated care for the paediatric population and adult population. It is a non-profit organization working in cooperation with the National Health System. We also have an IT departament working in support of the clinic.We have been part of the Advisory Committee   of the SWEET project (EU funded) and as a result of this we have been part of the launching of DOCE (the first national registry for type 1 patientes of paediatric age -< 21 years.APDP also cooperates in other multicentre projects in areas of diabetes prevention, educational, clinical guidelines, research (APDP/ERC - Education and Research Center), associative activities.APDP is the only national centre recognized by IDF, since 2009, as a Centre of Education of the IDF . In 2011 was distinguished as a Centre of Reference for Pediatric Diabetes and was considered, last June 2013, as an official collaborative Centre with World Health Organization (WHO).

 

P5 Fakultini Nemocnice v Motole (CZ) FNM

Univserity Hospital Motol, Prague, is one of the most important health institutions in the Czech Republic. The Centre of Pediatric diabetes of this hospital provides specialized care for diabetic children not only from Prague, but also from all over the country. In 2011, the centre was awarded by the title "Centre of Reference for Pediatric Diabetes". There is a long tradition of care for diabetic children at the Department, there is an experienced interdisciplinary diabetic team that includes physicians, educators, trained nurses, dieticians and psychologists.    

 

P6 P&A Kyriakou Children’s Hospital (EL) PAKCH

PAKCH’s Diabetes Centre is Greece’s largest pediatric centre, nominated as centre of reference on diabetes within the SWEET study. 450 patients visit the centre 3 monthly for follow up. 2 doctors, 3 nurses, 1 dietician, 1 psychologist belong to the centre’s staff & cooperate with the Depts. of Psychiatric Disorders, Social Services & Adolescent Medicine for difficult cases. Diabetes doctors, nurses from Athens University, and nutrition trainees attend the Centre and its educational programmes.

 

P7 University Childrens Hospital  (SI) UCH

It is with 160 beds the largest hospital in Slovenia. UCH is the central national institution committed to the diagnosis and treatment of children and young people from birth to age 18. In 2009 a new hospital was opened next to University Medical Centre. UCH is also the main training base for Faculty of Health Sciences and Faculty of Medicine in Ljubljana, which is housed nearby.
